New Patient Registration
If you live within our practice area (our practice boundry in 10 miles) you are welcome to register with us and our reception staff will be happy to guide you through the procedure. Eligibility can be quickly confirmed from your address so please provide proof by way of a recent utility bill along with one form of photographic ID.
You will need to complete a registration form and a health questionnaire which will provide useful information whilst we wait for your medical records to arrive from your previous doctor. Both forms are available on the above "Registering Online" tab.
All new patients are offered a health check (Over 16's only unless you have existing significant medical condition) with a Practice Nurse or one of our Clinican Assistants. You do not need to see a GP for this appointment. This check will ensure that any required tests are up to date and that we have an accurate note of any repeat medication you may be taking.
Medical treatment is available from the date of registration.

FAILING TO ATTEND
The practice operates a 'DNA - Did Not Attend' Policy - If you fail to attend 3 appointments within 12 months you will be removed from our list.
DRUG REDUCTION PROGRAMME:-
The practice is participating in a Drug Reduction Programme along with other practices in the area. If you are currently on any of the listed medications this will be reduced and then stopped.
Lyrica / Pregabalin
